|Tourniquet is too far from the site where it is to be needle stick !

Traditionally, proper clinical lab garb (lab coat with sleeve), brush-up hair , especially for infection control compliance on the wearing of clinical gloves is still prefered.*

She don't have the correct garb (lab coat to wrists, gloves), hair needs to be tied back, can't see any alcohol pads that she cleaned with, looks like a syringe- wrong needle, wrong angle of needle, tubes too far away, no band-aid or gauze too far away, and she didn't anchor the vein.
